Ejaculation is the peak of male orgasm, when the semen is released. For years, sex therapists have been arguing about the precise definition of 'premature' ejaculation. Do you know that Victorians sponged their armpits with sulphuric acid and in 1950s, you could be sedated with barbiturates or recommended for a course of x-ray treatment to help with this embrassing problem.For your info sweat is not smelly in itself, but it quickly becomes a breeding ground for bacteria. These bacteria break it down to produce fatty acids. It is these fatty acids which have the acrid, penetrating, pungent "stale sweat" smell. So the problem can be approached in two ways - sweating itself can be prevented, or the bacteria that cause the smell can be attacked.

How to help yourself

Commercial antiperspirants - you have probably tried most commercial deodorants and antiperspirants, but check the labels and look for one with a different active ingredient. Shave your armpits - hair holds sweat and gives the bacteria more to work on. Chlorhexidine 0.05% solution is an antibacterial liquid that you can buy from the pharmacy. Apply it daily. It won't prevent sweating, but will kill the bacteria. 20% aluminum chloride is the next thing to try if chlorhexidine hasn't done the trick. It can be bought from the pharmacist (ask for Drichlor or Anhydrol Forte). Use it as follows.

  • Before going to bed, wet and dry your armpits thoroughly. If you apply the solution to wet skin, a chemical reaction takes place that produces hydrochloric acid, which can irritate skin and tarnish jewelery.
  • Apply the solutions when you are lying down on the bed. This sounds odd, but armpit sweating switches off when you lie flat, and the solution will be more effective if applied them. The solution works by passing into the openings of the sweat glands, causing them to swell up and block, but if sweat is pouring out of glands when you apply the solution, it won't be able to get in.
  • Don't appy it directly afcter shaving, or the skin may become sore.
  • Wash off the solution in the morning, and don't reapply until bedtime.
  • If it proves effective reduce the apllication to every other night, and then to once or twice a week

The breast can enlarge if you are overweight, simply because fat has settled there. They can also enlarge because of development of the actual breast tissue; this is called gynaecomastia. To decide which it is, lie flat and grasp your 'breast' tightly between your thumb and forefinger. Then gradually move your finger and thumb towards the nipple. If you can feel a firm, rubbery disk-like mound of tissues which is more than 2 cm across, and which seems to be stuck to the back of the nipple and to the pink area surrounding the nipple (areola), it is likely that the breast tissue is over developed. The area may feel tender. Usually both breasts are equally affected, but sometimes develpoment of one is more obvious than the other. If there is no distinct mound of tissue under the nipple, it may simply be that you are too fat.

How breasts develop - Even a newborn has some basic breast tissue, which started to develop when it was a 6-week-old fetus. Before puberty, the breast are the same in girls and boys. They consist of tiny branching tubes embedded in packing tissues. The glands for making milk have not yet formed. At puberty, hormone levels start to rise. In men, the main sexual hormone is testosterone. This is made mainly in the testicles.In females, the main hormone is estrogen (estrogen makes the tubes thicken, lengthen and become more branched, and also stimulates the development of glandular lobules). Men normally also have a small amount of estrogen; at puberty it rises to only three times the level it was before. This estrogen often makes the breast tissue grow slightly in teenage boys, but eventually the high levels of testosterone take over competely and prevent the estrogen having any further effect on the breast. Therefore glandular lobules do not form in men. Instead, the breasts flatten out, and remain as a collection of tubes in packing tissue, just as they were before puberty.

Causes of breast enlargement in men - It seems that breast tissue is very sensitive to the balance between estrogen and testosterone in the blood. If there is either a fall in the testosterone level or a rise in the estrogen level, the breasts will be stimulated to grow. OBESITY is a common cause - if you are overweight, the breast will of course be larger because they are more fatty. In addition, fat produces oestrogen which stimulates breast development. DRUGS are the other most likely reason - Some drugs have an estrogen-like effect on the breast, and some block the effect of testosterone. Estrogens are easily absorbed throught the skin; men have developed breasts after using anti-balding scalp creams containing estrogen, and even by absorption through the skin of the penis from a partner using a vaginal oestrogen cream. ALCOHOL ABUSE upsets both sides of the estrogen: testosterone balance. It stimulates the liver to clear testosterone out of the blood, so testosterone level fall. It probably also reduces the ability of the liver to break down estrogens, so estrogen levels rise. Fortunately the liver can often recover if alcohol intake is reduced. TUMORS are rare causes of breast enlargement, Breast cancer can occur in the male breast, but is usually on only one side. Tumors in other parts of the body can sometimes produce hormones that make the breasts grow and may also cause impotence. If you have these symptoms it is vital to see your doctor right away.OLD AGE. It is natural for men's breasts to enlarge in old age. This is partly because less testosterone is produced. Also, in old age the body often contains a higher proportion of fat, which produces estrogens.

What your doctor can do

See your doctor if you think your breasts are enlarging, even if you have worked out the most likely cause. Your doctor will be able to:

  • Check whether the actual galndular breast tissue is overdeveloped, or whether the enlargement is simply fat
  • Check your testicles (because they make most of the testosterone)
  • Decide whether any drugs are likely to be responsible
  • Do blood tests to measure various hormones, including testosterone.

The treatment will depend on the reason. It may simply be a matter of losing weight or5 cutting down your alcohol intake. If the problem is a low testosterone level, testosterone can be given by injection or as patch. Tamoxifen - a drug that interfereswith the action of estrogen - is used by some specialists to reduce gynaecomastia. Another possibility is danazol, a drug that promotes the effect of testosterone. Danazol can have troublesome side-effects, such as weight gain, acne, muscle cramps and nausea. The excess breast tissue can also be removed by surgey.

It is normal to lose 50-100 hairs from the head each day. Each hair on the head grows for about 5 years before being shed. Eyebrow hairs grow for only 10 weeks. Scalp hair grows at a rate of about 1cm (just under half an inch) a month. We each have about 100,000 hairs on the scalp.

How hair grows

The portion of the hair that we can see is called the shaft. Each shaft of hair protrudes from its follicle, which is a tube-like pouch just below the surfaces of the skin. The hair is attached to the base of the follicle by the hair root, which is where the hair actually grows and where it is nourisehd by blood capillaries. Like the rest of the body, hairs are made of cells. As new cells form at its root, the hairs is gradually pushed further and further out of the follicle. The cells at the base of each hair are close to the blood capillaries, and are living. As they get pushed further away from the base of the follicle they no longer have any nourishment, and so they die. As they die, they are transformed into a hard protein called keratin, which is also the material of which fingernails, toenails and animals' hooves and horns are made. So, each hair we see above the skin is dead protein. It is the follicle, which lies deep in the skin, that is the essential growing part of the hair.

Hair growth is not a continuous process: it has several stages

  • The first phase is the growing stage. hair grows at about 1cm each month, and this phase lasts for anything between 2 and 5 years.
  • This is followed by a resting stage, during which there is no growth. This phase lasts for about 5 months, and is called telogen.
  • At the end of the resting phase, the hair is shed, and the follicle starts to grow a new one.
  • At any moment, 70-90% of the hair follicles of the scalp are growing hairs in the first phase; only 10-30% are in the resting phase.
  • If a follicle is destroyed for any reason, no new hair will grow from it.

How baldness happens

If any of the stages of hair growth are distrupted, the individual may become bald. For example, if the follicles shut down (meaning that they stay in the resting phase, and the shed the hair) instead of growing new hairs, there will be less hair on the head. Another reason mioght be interference with the formation of new hair cells at the root during the growing phase; this occurs with some anti-cancer drugs. If follicles have been destroyed (as they might be by, for example, a burn, or some skin diseases), there will be baldness in that area. An individual can also look if the hairs are growing but are so fragile that they break just as they emerge from the follicle. In 1998, scientists at Columbia University in New York announced the discovery of a gene that appears to be the 'master switch' for hair growth. They found the gene after comparing the genes of hairless mice belonging to a mutant breed, and comparing those of 11 members of the same family who had lost all thier hair. This discovery is a step towards understanding how the hair follicle works and how baldness happens, and may lead to effective treeatments becoming available within a few years.

How to know if you are losing too much hair - the tug test - Bald areas are an obvious sign, it can be difficult to tell wethere your hair is getting thinner. Hold a small bunch of hair - about 15 or 20 - between the thumb and index finger. Pull slowly and firmly. If more than six come out there may be a problem.

Tips to take care of your hair

  • It is not recommended to wash hair twice in succession, as often recommended by shampoo manufacturers. The first shampoo will wash your hair and the second will strip your hair of its nutrients. By repeating, you can irritate your scalp and overly dry your hair. HOWEVER, a long rinsing with water is a good habit, even if you are in a hurry; else, the hair will be dull and more sensitive to external irritants (pollution, etc.)
  • Use cold water as a final rinse; it will make your hair shinier. The cold water closes the scales that the hair has on its surface, which open when washed with hot water. Moreover, if your scalp tends to be greasy, cold water prevents dilatation of sebaceous glands and moderate sebum production.
  • Use mild shampoos if you want to wash often (they create fewer bubbles but this doesn't mean that they don't wash well; excess suds are more aggressive). Do not use children's shampoos, which are not soft but less galling; they contain over-greasing agents that make the hair more loose.
  • Use anti dandruff shampoos with care; they are more aggressive, can make hair less lively, irritate the scalp, and can actually increase the production of dandruff.
  • Dyeing of the hair can cause hair loss; although the products do not attack the root of the hair, they are aggressive to them and can harm the hair.
  • Brush carefully: strength of hair comes from the root; brushing will not give them more strength but can increase the creation of forks (split ends) and may uproot the hair. However, brushing may distribute the oils produced by the scalp throughout the hair, effectively acting as a natural conditioner and preventing oil buildup on the scalp. Be sure to use a brush with natural bristles, and never brush wet hair.
  • If you have forked hair (split ends), the only solution is to cut it and use a hydrating treatment to protect the new ends.
  • Play sports: Physical exertion increases perspiration, helping the skin to release toxins that may accumulate near the roots of hair on the scalp. Be sure to rinse your hair thoroughly after exercise to completely remove them from your hair.

Loss of erotic associations-The loss of libido can be a psychological process whereby the brain systematically re-labels stimuli which were once erotic as non-erotic. Physiological causes-Not all the causes of low sexual desire are psychological in nature. Low sexual desire in older men can be caused by a low level of the hormone androgen.
